Your application requirements
See what’s required to apply in your state by viewing the state specific information below. Then, complete the form at the bottom of this page to securely upload any required documents.
Illinois ITAC
To submit your ITAC application, the following document copies are required.
- Applicant's birth certificate (if under the age of 18)
- A recent phone bill (showing the phone company and phone number)
- Applicant's drivers license or state ID (if over the age of 18)
OR - Parent or guardians drivers license or state ID (the same parent/guardian who will sign the ITAC application)
Texas STAP
To submit your STAP application, one form of proof of address is required. If you don't currently have any of these items, our STAP program manager can review all the options with you.
Accepted forms of proof of address (please note, only one form of proof of address is required):
- Texas ID or drivers license
- Voter or vehicle registration
- Utility bill (gas, water, or electric)
- Letter on the official letterhead of a residential facility signed by the facility director or supervisor
Please note: your proof of address documentation must name the applicant OR the parent / legal guardian who is signing the application and show the home address as it appears on the application.

Wisconsin TEPP
To submit your TEPP application, the following document copies are required.

Ready to submit your documents?
You can upload your documents using the form below. Once you’ve selected your state on the form:
- If documents are required: You’ll see upload fields where you can securely submit PDFs or clear photos of your documents. All uploads are safely stored in our systems, and your program manager is notified as soon as your documents are received.
- If we’re not a vendor in your state: You won’t see any upload fields, and there’s nothing for you to submit. Instead, you can register your interest using our contact form, and we’ll reach out if we become a vendor in your state in the future.
