Once you complete this form:

1. We will complete an online application on your behalf, based on your responses to this form.
2. You will need to provide the last 4 digits of the applicant's SSN. You can include them on the form now, or share them with us later during the application process. Whatever works best for you.
3. TEPP requires a certification letter from the applicants provider: speech therapist, doctor, or teacher. We will provide sample letters to help in this process.
