Ready to start your application?

Provide some information about you and your speech impaired loved one and we'll send your pre-filled application for your signature.

Once you complete this form:

1. You will receive an email & text from us with next steps. For us to be able to give you updates about your application, please agree to receiving SMS texts from us.
2. Once we review your information, we may get in touch for a quick call to assess your eligibility.
3. SCEDP requires the following document copies to submit your application packet. These include proof of SC residency like a SC driver’s license or state ID, a recent phone bill, and a birth certificate (if under the age of 18). You can securely upload these now or later.
4. SCEDP requires certification from a licensed speech therapist. If you're working with a private or school SLP, you can provide their details in the form. If not, that's OK. We can organize a teletherapy appointment with a certified SLP at no cost to you.
