Who can I refer using this form?
You can refer any client or student who may benefit from our services and support. This includes individuals with communication, cognitive, or other developmental differences.

What happens after I submit a referral?
Our team will review the referral and reach out directly to their caregiver using the contact information you provide. If we require any additional information, we may also follow up with you. We will also keep you looped into their progress through the State program.
Should I let my client or student know I've summitted a referral?
Yes. We recommend informing them or their caregiver that you’ve submitted a referral so they’re expecting to hear from us.
Is individual consent required before submitting this form?
Yes. Please make sure you have the individual’s or their legal guardian’s permission to share their details with us.
