Provide some information about you and your speech impaired loved one and we'll send your pre-filled application for your signature.

Once you complete this form:

1. You will receive an email from us with next steps.
2. Once we review your information, we may get in touch for a quick call to assess your eligibility.
3. ITAC requires the following document copies to submit your application packet. These include proof of Illinois residency like a IL driver’s license or ID, a recent phone bill, and a birth certificate (if under the age of 18). You can securely upload these now or later.
4. ITAC requires certification from a licensed speech therapist. If you're already working with an SLP, you can provide those details in the form.
