Ready to start your application?

Provide some information about you and your speech impaired loved one and we'll send your pre-filled application for your signature.

Once you complete this form:

1. A family advocate will reach out by phone or text to walk you through the application process and answer any questions you have.
2. Later on, we'll ask you provide some documents required for to submit your application packet. These include provide proof of Illinois residency like a IL driver’s license or ID, a phone bill, and a birth certificate (if under the age of 18). You can securely upload these or send them to us via text or email.
3. ITAC requires certification from a licensed speech therapist. If you're already working with an SLP, you can provide those details in the form, otherwise we can help arrange a teletherapy appointment at no cost to you.
