Provide some information about you and your speech impaired loved one and we'll send your pre-filled application for your signature.
Once you complete this form:
- You will receive an email from us with next steps.
- Once we review your information, we may get in touch for a quick call to assess your eligibility.
- ITAC requires the following document copies to submit your application packet. These include proof of Illinois residency like a IL driver’s license or ID, a recent phone bill, and a birth certificate (if under the age of 18). We can help gather this information.
- ITAC requires certification from a licensed speech therapist. If you're already working with an SLP, we can guide you through the process. If not, that's OK. We can help you find an appropriate SLP to meet with.
